Clinical Studies of Henoch-Scho¡§nlein Purpura in Childhood

Abstract
Henoch-Schonlein purpura is a common pediatric self limited form of generalized by rsensitivity vasculitis involving the skin and to variable degrees joints, kidneys and gastrointestinal tract. Clinical manifestations and laboratory finding were observed and analyzed in 96 cases with Henoch Schonlein purpura who admitted to Saint Paul¢¥s Hospital during the period from April, 1978 to November, 1987.
The results are as follows
1. The age distribution of Henoch-Schonlein purpura shows a peak incidence between 5 to 9 years of age(59.4%). There was no significant difference in incidence between .hale and female.
2, Seasonal incidence was more common in fall and spring time.
3. Preceding illness was found in 45 cases(46.9%) of the total cases and URI was the most common one (40 cases, 41.7%).
4. The main clinical manifestations were skin manifestation(100%), abdominal manifestation (72.9%), joint involvement (53.1%) and renal involvement (19.8%) in order of frequency.
5. Skin lesions are present in all patients. The common types of skin rash were petechiae and purpura. Major sites of skin involvement are the lower extremities.
6. As for the gastrointestinal symptoms, abdominal pain was noted in 36 cases(51.4%), and associated with nausea and vomiting in 26 cases(37.1%). Intussusception was not o)served. (78.4%) (19.8%)
7. Joint most often involved were the knee in 40 cases (78.4 %).
8. The renal involvement was observed in 19 cases (19.8 %). The hematuria was see -I in 16 cases and the proteinuria in 5 cases.
9. The case leukocytosis over 10,000 / mm3 was observed" in 68.1% of cases and increases in ESR over 20 mm/hr in 50.6% of cases. Clotting studies, including bleeding time, clotting time, PT, PTT revealed within normal limit in most cases. Serum levels of kA ere elevated in 5 cases of 12 cases tested.
10. Among 96 cases of Henoch-Schonlein purpura, 10 cases- were recurred. One was the third attack
